OBJECTIVES OF POSITION

To provide advanced clinical leadership (RN Level 3) and governance for the Support at Home program, ensuring the delivery of safe, effective, and person-centred community-based care. The role leads clinical practice standards, oversees complex care planning, provides expert consultation to staff, and coordinates multidisciplinary input to support clients to live well at home. This position is responsible for mentoring and developing the clinical workforce, driving quality improvement, and ensuring compliance with regulatory, policy, and clinical governance requirements.

SUMMARY OF POSITION

Provide senior clinical leadership to the Support at Home program by guiding and developing a multidisciplinary team, ensuring Barunga Village’s services remain safe, sustainable, and aligned with the new Support at Home reforms. You will work collaboratively with clinical and operational leaders to uphold high standards of community-based care, drive continuous improvement, and ensure regulatory and clinical governance requirements are met. The role provides expert consultation, oversees complex care planning, and leads the clinical systems that support clients to live confidently and independently at home.

KEY RESPONSIBILITIES

• Assessment, Planning and Coordination

• Provide advanced clinical oversight for the assessment of clients with complex health, functional, cognitive and social care needs, ensuring evidence-based and person-centred practice underpins all decisions.

• Lead the development, review and evaluation of comprehensive Support at Home care plans, ensuring they align with consumer goals, risk profiles, and the requirements of the new aged care reforms.

• Oversee the coordination of multidisciplinary input, ensuring timely engagement with GPs, allied health practitioners, hospitals, assessment services and external providers to support safe and effective care at home.

• Provide specialist clinical consultation to Support at Home staff regarding deterioration, behavioural changes, risk escalation and complex case management.

• Ensure assessments and care plans incorporate safeguarding, reablement, wellness, and early-intervention principles consistent with contemporary community nursing standards.

• Support staff in identifying, documenting and responding appropriately to changes in consumer condition, including updating care plans and escalating to external health services when required.

• Oversee clinical recommendations relating to assistive technology, equipment and home modifications, ensuring justification meets funding and regulatory requirements.

Clinical Practice & Leadership

Provide expert clinical leadership to the Support at Home workforce, modelling advanced clinical reasoning, holistic assessment and high-quality community nursing practice.

Lead clinical decision-making and guide staff in delivering safe, goal-oriented and evidence-based care in a home environment.

Mentor, coach and support nurses and care coordinators to develop clinical capability, reflective practice and confidence in managing complex community-based clients.

Oversee the implementation of clinical pathways, best-practice frameworks and clinical protocols, ensuring consistent and high-quality care delivery across Community Services.

Act as a specialist clinical resource for the team, providing consultation on complex cases or emerging clinical concerns.

Build a positive, collaborative and learning-focused culture across the community team, aligning practice with Barunga Village values, the Aged Care Quality Standards and the Support at Home model.

Support the orientation, competency assessment and ongoing professional development of staff delivering services in the community.

Quality and Safety

• Lead the development, monitoring and improvement of clinical governance systems within Community Services to ensure safe, high-quality care.

• Oversee clinical incident management, including investigation, trend analysis and implementation of improvement actions.

• Monitor compliance with clinical documentation standards, assessment requirements, risk management processes and legislative obligations under the new Support at Home program.

• Support accreditation and audit readiness through robust clinical oversight, accurate reporting, and continuous improvement activities.

• Ensure risk is proactively identified and mitigated, including deterioration, safeguarding, home-environment risks, medication management and care-related hazards.

• Contribute to and lead quality improvement projects that strengthen outcomes for consumers living at home, ensuring changes are embedded sustainably into practice.

• Work collaboratively with operational leaders to support service sustainability, appropriate package utilisation and high-quality consumer experiences.

Senior Responsibilities

Provide senior clinical leadership within Barunga Village’s Community Services, representing the program in organisational governance forums and contributing expert advice on clinical risk, service design and quality outcomes.

Participate in strategic planning for the Support at Home transition, offering clinical insight into service needs, workforce capability, and risks associated with ongoing reform implementation.

Contribute to the development, review and implementation of policies, procedures and clinical guidelines relevant to community-based care.

Support staff recruitment, performance development and capability planning to ensure a skilled and confident Support at Home workforce.

Provide high-level input into service reporting, outcome measurement and program evaluation.

Build and maintain strong partnerships with external stakeholders including GPs, hospitals, allied health providers, assessment organisations and regulatory bodies.

Promote Barunga Village as a provider of high-quality community services, ensuring consumers and families understand available supports, pathways and opportunities for engagement.
